Claims Forms

Scotia Jamaica Life Insurance Company (SJLIC) administers both Death Benefit and Living Benefit claims. We are committed to approving claims within 3-5 working days, upon receipt of ALL required documents. In the event of an unavoidable delay, we will provide the necessary updates.

The following forms can be submitted via electronic means through our sjlic.service@scotiabank.com or through fax at 876-922-8675. (Please note that a previously signed Electronic Communication Agreement Form is required). They can also be printed and submitted to the SJLIC Insurance representative at your nearest Scotiabank branch.
